Preparing to Serve TennCare CHOICES Members
Haven Care Solutions is currently completing credentialing with UnitedHealthcare Community Plan of Tennessee to participate in TennCare CHOICES.
TennCare CHOICES provides long-term services and supports for eligible seniors and adults with physical disabilities who need assistance living safely in their homes and communities.
Credentialing is currently in progress. Haven Care Solutions is not yet representing itself as an approved TennCare CHOICES provider. CHOICES services will become available following required approval, contracting, authorization, and enrollment.
